Cigna PPO, East Bay Drayage, First Dental Health, Ameritas, Guardian, Humana, Blue Shield of CA, United Concordia, Aetna, MetLife, Anthem Blue Cross, Anthem (Laborers), Sun Life Financial, Dental Health Services, Premier Access, Principal Financial, GEHA Dentemax, UMR and more. When you come to the office, please bring your insurance card and forms. Insurance plans vary, and you are responsible for any fees not covered by your policy. Our team can help you complete and submit your insurance forms, but please address any questions or complaints about your coverage to your insurance carrier.
